Between May and October, approximately 100 admission forms were stolen from two San Francisco area medical centers. Summit Medical Center in Oakland discovered it had 70 missing forms and the Herrick Campus of Alta Bates Medical Center in Berkeley found it had 32 missing forms.

Information contained within the hospital admission records (i.e. names, home and business addresses, phone numbers, driver’s license numbers and social security numbers) were then used in a credit card scam through which thousands of dollars of merchandise was charged to patients.

Three arrests were made, but the investigation is continuing.
