License Number Format: First five letters of last name, first initial, middle initial, three numbers and two letters or numbers. Drivers retain number upon renewal.
License Appearance: Plastic laminated photo stock with “Washington” and state seal printed in gold within the laminate, overlapped by the department logo. The photo covers the right one-third of the document. For minor drivers, “Not 21 Until…” printed in a yellow bar.

License Appearance: A laminated card with state seal and state outline superimposed beneath text. Photo is located in the bottom left. For minors, a red background appears in the photo, and “Under 21 Until…” is printed above the birthdate.

Signature of director overlaps one side of the photo, which is located on the lower right. For minor drivers, the photo is placed on the lower left with “Age 21 in…” printed above it.
